I acknowledge that the services identified on this work order and any associated and necessary equipment have been installed or connected at my premises.  I have authorized the HiLight to attach equipment to the premises and make necessary connections to existing wiring, and I am authorized to order and install these services.  If I am renting or leasing the premises, the property owner has either granted me authority to authorize the installation of this service and equipment or I have provided HiLight with signed authorization from the property owner. I understand and acknowledge that the service(s) are provided on a month-to-month basis, payable in advance.  If I have agreed to a minimum term service package, I will be responsible for maintaining my service subscription throughout the required term.  If I disconnect prior to the completion of the term, I acknowledge that I will be responsible for repayment of the total service fee discounts retroactively to the beginning of the term. All rates are subject to change unless otherwise advertised as a term rate and applicable taxes and fees apply and are additional to these rates.  Until discontinued, I agree to pay for all services in a timely manner.  Unless purchased outright, all equipment installed by HiLight remains its sole property.  I accept liability for the full value of all equipment not returned upon discontinuation of service. 

I further understand that the acknowledgments set forth in this document either restate or supplement the terms and conditions in the Service Agreement published at HiLight Terms and Conditions, and that I have read, understand and accepted the Service Agreement.

Digital Voice Service Users:  By signing this form, I acknowledge that I have read, understood, and accept the limitations of the HiLight Digital Voice Service relative to emergency services & 911 dialing as described in the Digital Voice Disclaimer and Limitation of Liability, published at Digital Voice Disclaimer.

Installation with Temporary Drop:  I understand that HiLight and its Contractors may have temporarily installed the drop fiber and will bury the fiber on a subsequent premise visit.  HiLight and its Contractors have taken appropriate measures to secure and stabilize the temporary fiber drop cable.  However, regardless of those measures, I assume responsibility for myself and others who access my property, to avoid the potential hazards related to the fiber drop cable location.  I hereby release and agree to indemnify, defend and save harmless HiLight, its agents, officers, employees, volunteers and contractors from and against all claims, actions, causes of action, demands, judgments, costs, expenses and all damages of every kind and nature, incurred by and on behalf of any person whatsoever, predicated upon injury to or death of any person or loss of or damage to property of whatever ownership.
