Inquire with the Benefits & Leave team if you think you need to take leave and have a qualifying condition. HR will evaluate your eligibility and provide you the necessary paperwork to apply for leave.

Qualifying Leave Conditions include: 

1. Birth, adoption, or foster placement of a child 
2. Family member’s serious health condition 
3. Your own serious health condition 
4. Domestic violence, sexual assault, harassment, or stalking 
5. Sick child leave 
6. Military family leave 
7. Bereavement leave 
8. Public health emergency 

Leave Types & Duration

Depending on your situation, you may be eligible to take leave (1) continuously, (2) intermittently, or (3) on a reduced schedule.

Generally, FMLA, OFLA, and PLO provide up to 12 weeks in a 12-month period. Each leave for which you are eligible and qualified will run concurrently (at the same time), unless prohibited by law.

The City and the State of Oregon require you provide at least 30 days written notice, when the need is foreseen, when applying for leave. Or provide 24 hours verbal notice when the need for leave is unforeseen. HR will provide you the following forms: 

• City of Hillsboro Family & Medical Leave Request: Complete this leave request form and submit to HR to request medical leave and inform the City you have applied for Paid Leave Oregon.

• Medical Certification Form: The City must receive this form completed by the employee's or family member's physician before FMLA leave may be designated due to a serious health condition.

For FMLA and OFLA, completed forms are typically due back to HR within 15 days after you receive them from HR. If paperwork is delayed with the provider, please keep HR informed to discuss next steps. 

The City administers FMLA and OFLA. The Oregon Employment Department (OED) administers PLO, and you’ll need to apply directly with OED for PLO benefits. You’ll need to follow any additional application instructions provided by OED. 

FMLA/OFLA: After required paperwork has been reviewed and approved by HR, you and your supervisor will be informed of the leave status and anticipated dates of absence. Further instruction will be provided.  

Paid Leave Oregon: The Oregon Employment Department will notify the employee and the City when your application for Paid Leave Oregon (PLO) benefits has been submitted, and again after a claim decision has been made.

*Note: The City does not receive specific details about the circumstances of your PLO claim, so it is your responsibility to ensure HR knows the dates and the reason you are taking your PLO leave to ensure your leave is covered by all applicable protections.

You’re now on leave! Read carefully the communication you receive from HR and from Paid Leave Oregon. The Notice of Eligibility and Notice of Determination forms include your rights and responsibilities while on leave. You may be required to check in periodically with HR. You are not authorized to perform any work while on leave.  

Paid Leave Oregon: Follow any specific instructions you receive from OED. 

If your leave was due to your own serious health condition, you will need a return-to-work release signed by your treating physician. HR will discuss this with you if applicable.

If after your medical leave has ended, you are not fully released to return to regular work, the City will review your work restrictions to see if we are able to provide work for you within those restrictions.
